ENTRANCE REQUIREMENTS

Applicants for admission are required to submit the following credentials:

1. Freshmen

  • Report Card (Form 138) or Permanent Record (Form 137)
  • Certificate of Good Moral Character
  • Four (4) 1×1 colored pictures
  • Three (3) 2×2 colored pictures
  • Two (2) photocopies of Alien Certificate of Registration (ACR) for foreign students
  • Birth Certificate or Certificate of Naturalization for Filipino students

2. Transferees

  • Original copy of Certificate of Honorable Dismissal
  • Transcript of Records or Certification of Grades from the last school attended
  • Certificate of Good Moral Character from the last school attended
  • Four (4) 1×1 colored pictures
  • Three (3) 2×2 colored pictures
  • Two (2) photocopies of Alien Certificate of Registration (ACR) for foreign students
  • Birth Certificate or Certificate of Naturalization for Filipino students

3. Old Students

Previously enrolled students seeking re-enrollment shall submit their clearances along with the following:

  • Graded class cards of the previous semester
  • Evaluation for Grades from the Registrar
  • Student’s Clearance

4. Foreign Students

Foreign students are required to submit the following documents upon enrollment:

  • Original copy of the Certificate of Eligibility for Admission issued by CHED/TESDA
  • Original copy of Student Permit issued by CHED/TESDA
  • Photocopy of Alien Certificate of Registration (ACR)
  • Four (4) 1×1 colored pictures
  • Three (3) 2×2 colored pictures

5. Additional Requirements

5.1 Married Female Enrollees

Married female applicants must submit an authenticated photocopy of their Marriage Certificate in addition to all other requirements.

5.2 Entrance Examinations

All student applicants, including transferees, are required to take and pass the Entrance Examination administered by the Guidance Office as an additional requirement for admission.

5.3 Veterans Benefits

Students entitled to Philippine Veterans Administration educational benefits must present an eligibility certificate from the sponsoring agency in addition to the basic credentials.

5.4 Pre-Need Scholarship Insurance

Students enrolled in any accredited Pre-Need Scholarship Insurance agencies must present a certificate from the insurance company in addition to the basic credentials.
